The 2016 Leaders in Health Care Awards: Outstanding Achievement in Health Care Delivery

Challenging the status quo of health care delivery is what the CHI Franciscan Health Care team likes to do. Led by Matt Levi, director of Virtual Health Services, and Lana Adzhigirey, Care Transformation Progam manager, the team plays a critical role in implementing modern technological initiatives at CHI Franciscan.

When the team launched Virtual Urgent Care service in 2013, CHI became one of the first hospital systems in the United States to provide such service on a wide scale. The hospital found that patients tend to do significantly better when critical-care physicians are available and present 24 hours a day, seven days a week.

Another important initiative under the Care Transformation team involves using video consultations to improve patient access to specialists such as social workers, psychologists and neurologists. In January, the Care Transformation team unveiled a virtual urgent care kiosk at its Bonney Lake prompt care center.
CHI Franciscan also has launched a Virtual Diabetes Education program. The team is spearheading the organizations role as the first U.S. health system to use regional telemetry monitoring, which has enabled CHI Franciscan to monitor patients heart rhythms and rates proactively for more than two million hours.

Pacific Medical Centers (PacMed)
Seattle

PacMeds patient-centered approach has allowed it to exercise greater control over the quality and cost of health care services. The private nonprofit consists of nine clinics in the Puget Sound region affiliated with Providence Health & Services. PacMed uses electronic medical records and MyChart, an online portal where patients gain access to medical data, communicate with providers, request refills and make appointments. PacMed developed several programs, committees and reporting processes to ensure the effective delivery of quality care for patients.
