Provail began in 1942 to provide services to people with cerebral palsy and adopted its current name in 1999 to reflect service to a wider range of clients and a more entrepreneurial approach.

Provails fabrication division has more than 60 employees, who operate waterjets, CNC cutting machines and other equipment to produce parts for aerospace and other industries. It is a Silver Level supplier to Boeing, meaning it meets regular certification standards as a preferred vendor. In 2015, the fabrication division produced $3.6 million in revenue, a quarter of the organizations total. Profits from the fabrication division go to support other Provail programs, such as speech and occupational therapy, supported living and supported employment.

Provails headquarters and fabrication division had been situated across the street from each other in North Seattle, but the fab division recently moved to a new facility in Kent.
